Joined up human services - Lead Support Coordination Service request for proposals

The Hodgman Liberal Government is committed to improving services to deliver better outcomes for vulnerable Tasmanians.

Today we open the tender process to improve service delivery to people with complex needs.

Community sector organisations will be able to assist in the development of a Lead Support Coordination Service, which aims to test a service that works with individuals and families to provide tailored coordination of support needs.

Features of the service will include engaging and developing the client’s support network that has both client and service defined outcomes.

Advertisements seeking submissions appeared in today’s Tasmanian newspapers.

The service will be part of the ‘A hand up for vulnerable Tasmanians’ election commitment and continues the Government’s collaboration with the community sector on a common agenda towards person centred services.

Enquiries and documents can be obtained from Antony Deck, Joined Up Project, on (03) 6166 3672 or joinedup@dhhs.tas.gov.au.
